Using Your Health Insurance
I am happy to accept your health insurance and file your claim with your insurance company.
When planning to use your health insurance, it’s good to understand the details of your plan so there are no surprises. Please call the Customer Service phone number on the back of your card and ask these questions:
1. Do I need an authorization or pre-certification number?
2. Do I have to meet a deductible? How much is it?
3. If so, how much have I paid towards my deductible so far this year?
4. How much is my co-pay?
5. Are there any limits on the number of sessions per year? or per lifetime?
6. Any changes in my plan for the new year?
Current fees when working to meet your deductible or paying out of pocket are $65 per 55-minute session. Your co-pay will be as determined by your plan. Please plan to pay, at each session, with cash or check.
